Almost every week, the FDA removes tainted dietary supplements from the market due to health concerns. For example, in January 2016, the FDA issued an alert about “Pink Bikini and Shorts on the Beach Capsules” from Lucy's Weight Loss System of Arlington, TX. It was found that these weight loss aids contained sibutramine, phenolphthalein, and/or diclofenac, but these ingredients were not declared on the label. Sibutramine can cause increase blood pressure and/or pulse rate in some patients and may present a significant risk for patients with a history of coronary artery disease, congestive heart failure, arrhythmias, or stroke. Phenolphthalein was previously used in over-the-counter (OTC) laxatives, but was removed from the US market. Health risks linked to phenolphthalein could include potentially serious stomach upset, irregular heartbeat, and cancer with long-term use. Diclofenac is a nonsteroidal anti-inflammatory drug (NSAID) which could lead to serious side effects in patients taking other NSAIDs, with recent heart surgery, or with allergies to NSAIDs. NSAIDs can also lead to stomach bleeding, fatal heart attack or stroke.

Newer prescription weight loss drugs such as Qsymia (phentermine and topiramate), Belviq (lorcaserin), Contrave (bupropion and naltrexone), and Saxenda (liraglutide) are indicated for chronic, long-term use, but only if adequate weight loss (as outlined in the package labeling) occurs. If a 3 to 4 percent weight loss has not been achieved after 12 to 16 weeks, long-term treatment is typically stopped.

For decades, the usual doctor spiel is to get 150 minutes a week of moderate exercise. But that hasn’t really translated into any meaningful changes nationally, has it? So here’s where another trendy (uh oh) regimen is gaining popularity, mostly because the growing research is impressive. It’s called High Intensity Interval Training (HIIT), and it basically means you go all out for 30-60 seconds on any activity, getting to maximum exertion, then take a few seconds break, then do another all-out effort, etc etc. You do this 15 minutes tops, twice a week only (typically). A simple way to slim down: Eat the rough stuff. Simply adding more fiber to your diet can lead to weight loss. That’s because your body can’t break down fiber, so it slows down digestion and takes up space in your stomach. With 4 grams of fiber per cup, oatmeal can be a good source. Walnuts can add about another 2 grams, plus satisfying protein and crunch.
As my colleagues have reported (here and here), when it comes down to it, it’s not the body or the metabolism that are actually creating overweight or obesity – it’s the brain. We all know intuitively that poor decisions are what make you gain weight and better ones are what make you lose it. The problem is that over time, the poor decisions lead to significant changes in how the brain governs – and, amazingly, responds to – the hunger and satiation processes. Years of any kind of behavior pattern lay down neural tracks, and overeating is no exception.

There’s a lot of research that coffee is fine. Coffee’s been redeemed.” The Mayo Clinic goes even further, saying: “Caffeine may slightly boost weight loss or prevent weight gain.”

Keep researching. I’ve recovered from sciatica pain, lost 19 lbs, dramtically reduced my cholesterol and lowered by blood pressure from 137/93 to 125/89 in 3 months ! Read Dr Greger’s book: “How Not to Die” and Dr Campbell’s book: “The China Study.” Watch the documentary “Forks Over Knives.” Look up Dr Ornish, Dr Barnard and Dr Esselstyn. I’m convinced that eliminating all (or almost all) animal protein from your diet is the way to go. There’s a mountain of evidence that shows a whole food, plant based diet can prevent many cancers including prostate and breast cancer and even reverse many major diseases like heart disease (Dr Esselstyn). Dairy is very bad for you, your patients and your kids. But, don’t take my word for it, you’re a doctor, do the research. Dr Greger’s book references thousands of studies. May be the best place to start.
